Check out this Zack the elephant , made by Boyd's Glass, located in Cambridge, OH.  This is a mould that the Boyd's had made for them and has been made in various colors.   Marked on the side with the B in a Diamond, with 4 lines and a 2.

It measures almost 4 inches high and 4 inches long.
