This is a "Schoolmate IIe" add-on card for the Apple II series of computers.  It consists of a fairly complex expansion card that plugs into the Apple's expansion card bus, and a small enclosure with four pushbuttons, two LEDs, and a DB9 on the back.  (There is no additional circuitry in this box).

I have no idea how this is used, but it looks like a simple networking and/or printer sharing system for Apples.
