 My girlfriend received this from a training session she attended. She works in a cosmetic store in a major casino in Vegas. 
This is brand new and comes with a leather or vinyl pouch (not sure about the material). It is marked not for resale, I guess they use this size for testing or for give aways when you purchase something else.
 
The size is .11 oz.
Color is Medium-02
Concealer
This doesn't have a pad with it.
Retails for $65.00 I'm told.
