The forms are for recording genealogical information for a family, including marriage and death information for parents and their children. The forms also have places to record temple ordinances. The vertical format is the primary form for submitting names of deceased people for temple work.
These are great for taking with you as you go places to collect your information as you can enter notes on the backside of the form, and by referencing to each entry on the front of the form you'll now be able to properly enter each event in your PAF (Personal Ancestral File) program (or any other program you use to record your family history).
