With Bag, Box, and Paperwork.  This was a gift and it is a great helmet, but does not fit me well.  My chin is too big!!!  I have worn this twice, never dropped.  I am very careful with all my equipment. It is sitting in the bag, in the box on a shelf in my garage, waiting for a new home!
