This is the 2003 Longaberger Classic Horizon of Hope basket combo complete with Horizon of Hope Scalloped Liner, Protector, WoodCrafts Lid and Pin/Tie-On.  It has the American Cancer Society logo burned of the bottom along with Longaberger's.

I'm including the flyer that it was in. 

Smoke-free/Pet-free home.  It measures 5" x  4 x 2" .
