Vintage 8-Bit Graphics I/F card by RADIUS.
I believe this is for the old 1 page or 2 page black and white monitors that they had a long long time ago. I don't even recall what Mac's this works with... If there's anything you can ask me or include about this card, please use the link on the right-hand side "Ask the seller a question" and let me know, I will post it.
