IT STATES: "I suffer from Obstructive Sleep Apnea. When Sleeping, I use a CPAP/BIPAP, (Continuous Positive Air Pressure), Machine at all times. If Unconscious my air way may become obstructed. (Turn card over for medical history)

Important information  such as the type of equipment you use, CPAP / bi-level /auto-titrating machine. Below is a place to write in your pressure settings. This information is located on the front of the card. "Keeping you breathing is number one priority in keeping you alive"!

On the back of the card there is room for: Blood Type, Conditions / History, Medications, Allergies, Emergency Contact, Phone  No., Physican's Name, Phone No., Date and a place for your signature. All the necessary information you need to supply, in case of emergency.

The card size is 3 1/4" X 2 1/8". This is credit card size to easily fit in your wallet or purse. Once you have filled in all of the necessary lines on the card, you should date and sign it to make it an offical document for emergeny use.  You may choose, even to have the card laminated.
