This is a big one measuring 2-1/4 inches tall and 1-7/8 inches wide.
The Cameo itself is 2-inches by 1-1/2 inches.

Beautiful intricately carved hair tendrils and facial details.
Likely carved of conch shell.

ISSUE:  Top of the head near the floral wreath on her head are a series of lines.
I think they are tiny hairlines that are showing from the back of the cameo.
Can be see in image above.

The reverse side of the shell shows some lines radiating from the setting.
There is also an area of lighter color shell that follows the curve of the setting near these lines and a couple of nips to the shell.
In my NON-professional opinion, I would conclude that this portion of the brooch was stressed in some way, resulting in the tiny radiating lines and nips to the shell.
None of the lines or nips go all the way through the piece. 
The issue I am describing can be seen in the images above and below.

I believe the setting is gold filled, though it has not been tested.
There is tarnish along the right and left outer lip setting.
I also see brass color near the seams where the metal pieces are soldered together.

There is a very very small nip on the bridge of her nose, less than 1mm in size.

There is also an area of lighter color shell and a nip/small line (about 1mm in length) that follows the curve of the setting near the clasp, seen in image below.
