This item is the Simplicity sewing pattern #9009 shown here. The pattern was copyright in 1979.

The pattern is uncut, unused and still in factory folds. The instructions are in the envelope. The back of the envelope is torn - does not affect contents.

The pattern is a Men's size 40: Chest 40"; Neck 15½".

The pattern description reads as follows:  "Men's Shirt:  Topstitched shirt with yoke forming forward shoulder seams has back pleat and loop, stand-up collar, set-in sleeves and shirt-tail hem. Views 1 & 2 have front band closing, pleated pocket with band, and long sleeves with buttoned cuffs. View 2 has contrasting collar, pocket band and cuffs. View 3 with front tab button closing has short sleeves."
