In order to register your dog as a Service Dog you merely need a Doctor to sign this form and then to get your Veterinarian to sign the same form.  The Doctors signature is the same as a prescription authorizing you to have a Service Dog.  The Veterinarians signature states that your dog has had all of it's shots and that the dog is not vicious and/or a threat to society.

Dogs can be registered for other services than just for the blind and the deaf.  Many doctors prescribe dogs for anxiety, psychiatric, and other general health issues.  Most owners also train their dogs themselves for their own needs.

Under the rules of "The Americans With Disabilities Act" you are NOT required to have identification for your dog however, most business owners are not familiar with this law and that is why it is important to have identification cards with you.  Your life with your dog will be much easier with the proper identification papers.

Do you travel?  The airlines recognize my registration card!  Also, you do not have to pay extra to have your dog with you in a hotel/motel.  You will not have any trouble taking your dog into a restaurant with this ID.

Many people purchase this registration because landlords will not allow a dog in your residence unless you can prove to them that the dog is indeed a Service Dog.
