provides a total of $150 for car rental and a total of $100 for towing.

Replacement Parts: In case of a failure of a part as listed above, the

EasyChoice. product. Worn or burned parts are not covered. Parts will
be replaced with like kind and quality based on the administrator..s
discretion. Transferability: This treatment program is transferable
to a second owner of the vehicle or to a replacement engine and/or
transmission, provided the administrator listed below is notified
in writing, all maintenance records are reviewed prior to the transfer,
and a $50 transfer fee is paid. Please direct questions regarding
transfers to transfer@warrantyadminservices.com. Refund
Procedures: This program is not cancellable and is refundable only in
the event of a verifiable repossession or an insurance total loss. All
refunds will be calculated by a time and mileage factor from the
inception date, less a $50 processing fee. Any claims paid prior to
refund date will be deducted from the remuneration. Please direct all questions regarding refunds to the seller as listed above.
Administrator..s Right to Terminate Benefits: In the event of filing a
claim with administrator, the administrator reserves the right to
terminate the benefits of this program upon the discovery of fraud,
misrepresentation of a material fact by the vehicle owner, or the
vehicle owner..s representative. Evidence of fraud or misrepresentation
will be forwarded to the proper state and federal authorities. Any vehicle used in the commission of a crime will have all benefits terminated.
Purchaser Rebate Guarantee:, The administrator will refund the retail
purchase price of this product or $1,500.00, whichever is less,
provided that the administrator has received this registration and
full payment subject to the terms and conditions that are as follows:
Program benefits must remain in force for the full term and the covered
vehicle must be registered to the original purchaser at the
expiration date and mileage of the program. The PRG benefits are
payable only to the original purchaser as shown on this registration
form and are NOT transferable to any other party. To be eligible for
PRG benefits: this program must expire by both time and mileage and, no
claims or benefits may have been paid on this program, the original
purchaser must provide acceptable documentation that all installation
and maintenance requirements as stated on this agreement have been
adhered to and, the original purchaser must file for PRG benefits
within 30 days or 200 miles after the expiration of this program by

agreement, documenting the retail price you paid for the program. 4. A valid and current copy of the vehicle registration showing that you still own the vehicle. 5. All product installation and vehicle maintenance records associated with the above listed vehicle.
Eligible refunds will be processed and mailed directly to the Purchaser
within 30 days. The vendor or selling dealer is not an obligor or party
to the Purchaser Rebate Guarantee. ?BENEFIT ACTIVATION: After the delivery of the benefit activation kit to the above listed purchaser:
1. It is the responsibility of the purchaser of this program to have

a commercial lubrication service facility within fourteen days (14

handwritten information on receipts, records or repair orders,

exceeded 250,000 miles or 10 years have elapsed from the sale date of

resulting from: abuse, negligence, freezing, overheating of any type,
failure to maintain proper coolant and fluid or lubrication levels,
personal damages, per diem expenses, storage fees, medical
expenses, telephone or rental charges, fire, flood, vandalism, theft,
collision, acts of God, racing, usages not approved by the vehicle
manufacturer, improper load capacity, or improper towing, commercial
usages, damages caused by contaminated fluids or lubricants for any
reason, misuse, road conditions, alterations not approved by the
vehicle manufacturer, riots, or acts of war, diesel engines
manufactured prior to 1990, rotary engines, excessive oil consumption
and diminished performance is not covered. Parts or components that
have been damaged by excessive heat or scoring are not covered.
Electrical components are not covered by this program. 4.
Pre-existing conditions and problems that occur prior to the benefit
activation date and mileage are not covered. Parts that are worn or
burned but NOT broken are not covered. Broken parts are defined as parts that have cracked or separated into pieces.
5. Any component or part not listed as a covered part will not be
covered. Incidental and consequential damage is not covered by this
treatment program. 6. Any vehicle with a non-functioning odometer
is not eligible for this program. Claims occurring outside the United
States are not covered. 7. Any work or repair done to your vehicle
without prior written authorization from the administrator will not be
considered a valid claim. 8. Any claim filed when a manufacturer..s
warranty or extended service contract is in force on the above listed
vehicle will not be eligible for coverage under this program. Limit
of Liability: per single claim is $4,000 or the actual cash value of
the vehicle, (wholesale value) as determined by the Kelly Wholesale
Blue Book or the NADA Wholesale Guide whichever is the lesser
amount at claim time. The total of claims paid shall not exceed the
actual cash value, (wholesale value) or the vehicle as determined by
the Kelly Wholesale Blue Book or the NADA Wholesale Guide. HOW TO FILE A CLAIM: PLEASE FOLLOW THESE INSTRUCTIONS CAREFULLY BEFORE STARTING WORK ON THIS VEHICLE!
1. Take your vehicle to an ASE certified service facility and have them
call 877-554-9863 to begin a claim. Claims can be initiated by the
repair facility electronically by logging on to
www.warrantyadminservices.com and selecting the claims button. Be sure
that no work is started until proper authorization is received.
Reasonable access must be provided to the vehicle for inspection. 2. Acceptable maintenance records and a copy of the completed vehicle inspection checklist will be required to file all claims. 3.
