This is a Vivitar model number 0821623.  It fits Canon, Konica, Minolta cameras.  It is a 70-210 mm f4.5 Focusing Zoom Len.  Included is the paperwork and the pouch and the lens cover.  I dont know much about this I found it at an estate sale.  It looks like it has never been used or used very little.
